General Description 

The Nurse Home Visitor provides preventive health and social services to at-risk pregnant women and children from birth to age two. Using advanced nursing skills, the Nurse Home Visitor assesses family needs, screens clients for eligibility, delivers evidence-based educational interventions, provides case management, and makes referrals to community resources to support positive family functioning and child outcomes. The Nurse Home Visitor collaborates closely with supervisors, community agencies, private providers, and the health department, delivering services in homes, hospitals, and clinics. All work is conducted in a safe and professional manner, with the goal of achieving family outcomes that meet or exceed evidence-based standards.

Essential Duties and Responsibilities 

1. Client Engagement and Therapeutic Relationship Building

  • Establish andmaintaina therapeutic, trust-based relationship with pregnant individuals and families experiencing poverty. 
  • Engage families from early pregnancy through program completion to support positive health and life outcomes. 
  • Provide consistent, culturally responsive, and family-centered care. 

2. Home Visitation and Direct Nursing Services

  • Conduct regular home visits with eligible pregnant individuals and their familiesin accordance withprogram guidelines. 
  • Deliver comprehensive nursing servicesutilizingthe nursing process, including assessment, planning, intervention, and evaluation. 
  • Manage a caseload ofapproximately 25–30 families, ensuring consistent and high-quality service delivery. 

3. Health Education and Parenting Support

  • Provide education, guidance, and support to first-time parents on topics including pregnancy, childbirth, and postpartum care. 
  • Educate families on child growth and development, health, safety, and age-appropriate parenting practices. 
  • Offer practical instruction andevidence-based information to promote healthy outcomes for both parent and child. 

4. Family Empowerment and Advocacy

  • Empower families to build self-sufficiency, confidence, and decision-making skills. 
  • Support families in developing the ability to advocate for themselves and their children within healthcare and community systems. 
  • Encouragegoal-settingandassistfamilies in working towardidentifiedpersonal and family goals.

5. Care Coordination and Community Resource Navigation

  • Assess family needs and connect families to appropriate community resources and support services. 
  • Collaborate with healthcare providers, social service agencies, and community partners to ensure coordinated care. 
  • Cultivate andmaintainpartnerships that support families and strengthen program impact. 

6. Assessment, Monitoring, and Documentation

  • Conduct ongoing assessments to evaluate maternal and child health, development, and family progress. 
  • Monitor outcomes in alignment with program goals andevidence-based benchmarks. 
  • Completeaccurateandtimelydocumentation of all visits, assessments, and services within 24–48 hours. 

7. Program Fidelity and Quality Assurance

  • Adhere to the established model of the Nurse-Family Partnership (NFP) program, ensuring fidelity to all core components. 
  • Participate in ongoing training, supervision, and reflective practice tomaintainprogram quality. 
  • Ensure services are delivered in alignment with organizational standards, policies, and best practices.

Essential Qualifications  

Education/Experience Requirements:

  • Bachelor of Science in Nursing is required.
  • Current license or temporary license to practice as a Registered Nurse in North Carolina by the NC Board of Nursing is required.
  • At least one year of Public Health experiencepreferred;or graduation from an accredited school of professional nursing with at least two years of professional nursing experience, including one year of public health experience.
  • Maternal Child Health nursing experience is preferred. 
  • Spanish as an additional language is preferred.

Knowledge, Skills, and Abilities:

  • Current, valid North Carolina driver’s license isrequired.
  • Certification in basic CPR

Physical, Mental and /or Visual Demands

  • Must be able to sit and stand for extended periods of time throughout the day.  
  • Must be able tolift upto 35lbs.
  • Must be able to bend and stoop.  
  • The noise level in the work environment is usually moderate.  
  • Specific vision abilities required by the job include close, distance, color, peripheraldepthand the ability to adjust focus.
