Harbor’s Edge is a premier 5-star Continuing Care Retirement Community (CCRC) located on the Elizabeth River in Norfolk, Virginia. Our community offers a full continuum of care, including Independent Living, Assisted Living, Memory Support, and Skilled Nursing services.

We are seeking highly qualified, part- and full- time Licensed Practical Nurses (LPNs) to join our team to deliver excellent care for our residents. The ideal candidates will have senior living experience (e.g., long-term care, assisted living, memory support) with a passion for working with an elderly population.

CURRENT SHIFTS AVAILABLE:

  • 7:00 AM - 3:00 PM
  • 3:00 PM - 11:00 PM
  • 11:00 PM - 7:00 AM

KEY RESPONSIBILITIES:

  • Provide direct nursing care per care plans across CCRC levels of care (independent living, assisted living, skilled nursing), supporting physical and psychosocial well-being.
  • Perform assessments, document findings, and report condition changes to RN/physician and care team.
  • Monitor vital signs, weights, I&O, and blood glucose; administer medications, injections, wound care, and ordered treatments within scope.
  • Implement restorative care, including positioning, ADL training, fall prevention, and range of motion exercises.
  • Assist residents with ADLs (bathing, dressing, toileting, grooming, transfers, and ambulation) using safe techniques.
  • Support engagement in recreational, social, and therapeutic activities to promote quality of life.
  • Coordinate safe admissions, transfers, and discharges within the CCRC, ensuring continuity of care.
  • Collaborate with interdisciplinary team members to develop and update individualized care plans.
  • Educate residents and families on medications, disease management, and health maintenance.
  • Follow infection control standards, safety regulations, and facility policies.
  • Maintain accurate and timely EHR documentation of care, assessments, and outcomes.
  • Support and guide CNAs and support staff as needed to ensure quality resident care.

Requirements

REQUIREMENTS:

  • High school diploma or GED required.
  • Current Virginia LPN license required.
  • Current Adult CPR and BLS/AED certification required.
  • Minimum of three (3) years of progressive experience in CCRC, long-term care, home health, case management, or related healthcare setting.
  • Strong commitment to service excellence, resident safety, and high-quality care.
  • Excellent communication, organization, time management, critical thinking, and multitasking skills.
  • Self-motivated with ability to follow verbal and written instructions accurately and efficiently.
  • Compassionate, patient, and professional in providing care to geriatric residents.
  • Respects resident rights and maintains confidentiality and discretion with protected health information.
  • Knowledgeable in nursing standards, clinical procedures, safety protocols, laws, and regulatory guidelines.
  • Able to work independently and exercise sound clinical judgment in routine and urgent situations.
  • Proficient in EMR systems, Microsoft Office, and standard clinical/office technology.
  • Able to meet schedules, deadlines, and reporting requirements consistently.
