Licensed Practical Nurse (LPN) – Personal Care
We are seeking a compassionate and dedicated Licensed Practical Nurse Supervisor (LPN) to join our team, providing strong supervisory skills to our Med Tech's and CNA's in our Personal Care Community. This role offers an opportunity to make a meaningful difference in the lives of residents requiring assistance with daily activities, ensuring their comfort, safety, and well-being in a supportive environment.
Bi-weekly Schedule is Mondays, Fridays and Every other weekend 7a-7p and available pick up time if desired.

Key Responsibilities:
- Supervising the care team of MED TECH's, CNA's and Direct Care Workers to ensure excellent care is given to residents
- Assessments, Admissions and Discharge of residents as needed
- Monitor and record residents' health status, vital signs, and any changes in condition
- Administer medications and treatments as prescribed by healthcare providers
- Support residents with activities of daily living while promoting independence and dignity
- Maintain accurate and detailed documentation of care provided and resident progress
- Collaborate with healthcare team members, physicians and therapists to develop and implement individualized care plans
- Ensure a safe, clean, and comfortable environment for residents at all times

Requirements
Skills and Qualifications:
- Valid and current Licensed Practical Nurse (LPN) license in Pennsylvania
- Proven experience in a personal care setting or skilled nursing with at least 2 years of supervisory experience is required
- Strong interpersonal and communication skills
- Compassionate, patient, and respectful attitude towards residents
- Ability to follow detailed instructions and adhere to safety protocols
- Good organizational skills and attention to detail
- Ability to work independently and as part of a team,
- Ability to lead a team, make emergent decisions and follow company policies and procedures
- Physical ability to assist with mobility and perform caregiving tasks
