About the Role:
Clinical Care Transitions Coordinator is a unique role for a clinically licensed professional who enjoys building strong physician relationships and expanding referral networks. In this field-based role, your primary responsibility will be to promote and sell our Home Health Specialty Programs to physicians and acute care settings.
You'll market our specialty programs directly to physicians and other healthcare providers, combining your clinical knowledge with relationship-building skills to grow strategic partnerships and increase patient referrals.
Essential Functions:
- Drive referral growth by promoting and selling our Home Health Specialty Programs to physicians, hospitals, and acute care providers while building trusted, long-term referral relationships.
- Promote our specialty programs by educating physicians and referral sources on our clinical services and patient care solutions.
- Serve as a trusted clinical resource, confidently communicating with physicians and healthcare professionals using appropriate clinical terminology.
- Conduct in-person visits to physician offices and healthcare facilities to develop and maintain strong referral partnerships.
- Collaborate with clinical and operations teams to ensure a seamless transition of referred patients into our services.
- Stay informed on our specialty programs, clinical offerings, and industry trends to effectively represent the organization.
- Represent the organization professionally at networking events, provider meetings, and other business development opportunities as needed.
Requirements
- Active RN, LVN, PT, or PTA license required.
- Experience in homecare, home health, hospice, and/or assisted living strongly preferred
- Previous physician liaison, clinical marketing, healthcare sales, or business development experience preferred.
- Excellent communication and presentation skills, with the confidence to engage physicians and healthcare professionals using appropriate clinical terminology.
- Flexible, hands-on, and willing to step in wherever needed
- A caring professional who leads with empathy, integrity, and follow-through
- Valid driver's license, reliable transportation, and willingness to travel within the assigned territory.

Payrate: $75,000 to $85,000 based on experience and license plus bonus plan eligibility.
