About the Role


Job Title: Admission Nurse (LVN) – Skilled Nursing Facility (SNF)

Job Summary:
We are seeking a compassionate and detail-oriented Licensed Vocational Nurse (LVN) to join our Skilled Nursing Facility as an Admission Nurse. The Admission Nurse is responsible for coordinating and completing the admission process for new residents, ensuring accurate assessments, medication reconciliation, physician orders, and seamless transitions from hospitals or other care settings. The ideal candidate has strong clinical assessment skills, excellent communication, and knowledge of long-term care regulations.

Key Responsibilities:

  • Complete comprehensive admission assessments and nursing documentation.
  • Review hospital records, physician orders, medications, and treatment plans.
  • Perform medication reconciliation and verify admission orders.
  • Coordinate admissions with physicians, case managers, hospitals, and interdisciplinary team members.
  • Conduct head-to-toe and skin assessments upon admission.
  • Ensure all required regulatory and facility documentation is completed accurately.
  • Educate residents and families regarding the admission process and plan of care.
  • Communicate changes in resident condition to the supervising RN or physician.
  • Document all admission activities in the electronic health record (EHR).
  • Ensure compliance with federal, state, and facility policies and procedures.
  • Collaborate with therapy, dietary, social services, and other departments to promote continuity of care.

Qualifications:

  • Current LVN/LPN license in good standing.
  • Experience in skilled nursing, long-term care, rehabilitation, or post-acute care preferred.
  • Knowledge of Medicare, skilled nursing regulations, and EHR documentation.
  • Strong assessment, organizational, and communication skills.
  • Ability to prioritize multiple admissions and work independently.
  • Current CPR/BLS certification.
