Wage: $21.22 - $22.28 depending on qualifications

Position Summary
The purpose of this position is to provide Housing Support and Mental Health Support to people with housing instability and/or mental health instability.
Responsibilities:
- Lead with a service focus – Set the example for and lead others in focusing on providing excellent service for people served, their families, and all other people who support them; etc.
- Communication – Speak clearly; listen to others; write effectively; establish relationships; act with integrity; establish trust
- Professional Involvement- Participate in professional activities (civic and/or trade associations, professional conferences, etc.) and monitor industry trends and activities. Pursue opportunities to present program information and expertise. Work effectively with external stakeholders and industry partners. Serve as a liaison with other appropriate agencies and organizations to promote the interests of Rise, Inc.
- Assess functioning levels, strengths, needs and vulnerabilities of people served to create a functional assessment plan using the individual's participation
- Develop and implement ARMHS Treatment Plan reflecting needs identified in functional assessment
- Provide access to referrals, outreach, coordinate and authorize funding for services including medical/psychiatric, vocational, social, educational, financial, legal, housing and chemical dependency
- Advocate for individual to ensure they are receiving the appropriate services for which they are eligible, in need of and requesting, particularly when communicating with other service providers, family members and other people
- Complete and maintain accurate case records, reports and paperwork in a timely manner
- Complete all required annual trainings on time using Rise’s online learning management system.
Position Requirements:
- Must meet the qualifications below required for Mental Health Practitioners under MN DHS regulations:
- BA/BS in behavioral science or related field AND
- 2000 hours supervised experience delivering direct services* to persons with MI OR
- Fluent in non-English language or which 50% of the clients served speak
- 40 hours training in delivering services to persons with mental illness AND
- Clinical supervision from MH professional once per week until 2000 hours experience
OR One of the Following
- 4000 hours supervised experience delivering direct services* to persons with mental illness
- Graduate student in behavioral science or related field and assigned to Rise for clinical training
- Masters/other graduate degree in behavioral science, less than 4000 hours post-masters experience
- Strong self-management, thinking and interpersonal skills
- Ability to work with diverse groups including team members, people served, community partners and internal management
- Excellent time management and multi-tasking skills with strong ability to problem solve
- Ability to direct and motivate others, perseverance and compassion for team members and the people we serve
- Technology skills: Proficient with Microsoft Office software (Outlook, Word, etc.). Case management (MyEvolv) – Learn software system and become proficient • Current, valid driver’s license
- A driving record that demonstrates safe, legal driving habits
- Compliance with Rule 11 and MVR background check required
Preferred qualifications
- 2 years delivering Mental health supports preferred.
- Previous experience practicing person-centered practices with people with disabilities or other barriers to housing.

Working Conditions and Environment:
- Hours of work are typically daytime, Monday through Friday
- Must be able to travel to various locations throughout the day including the ten county service delivery area of Stearns, Sherburne, Benton, Wright, Mille Lacs, Isanti, Chisago, Kanabec, Anoka and Mora.
- This job operates in a variety of environments. The employee in this role will meet with people supported in a variety of locations, such as coffee shops, Rise office, person supported’ s home, etc. The employee will also meet with prospective and current employers of persons supported
- The team member frequently is required to twist, stoop, bend, reach, pull, push, stand, sit and walk for up to eight hours per day
- Frequently required to sit (or equivalent) for up to eight hours per day
