Our home health census is growing, our therapy team is expanding, and we’re looking for talented Physical Therapists (PTs) and Physical Therapist Assistants (PTAs) who know how to make the magic happen in the home.

💙 What You'll Do

As a member of our therapy team, you’ll help patients regain strength, improve mobility, reduce pain, prevent falls, and return to the activities that matter most to them.

PTs will:

  • Complete comprehensive evaluations and identify functional needs.
  • Develop individualized plans of care and treatment goals.
  • Provide evidence-based interventions and monitor patient progress.
  • Reassess patients and modify treatment plans as clinically appropriate.

PTs & PTAs will:

  • Provide skilled therapy in accordance with physician orders and CMS Conditions of Participation.
  • Deliver therapeutic exercise, gait and balance training, transfer training, strengthening, and functional mobility interventions.
  • Educate patients and caregivers on fall prevention, assistive devices, safety, and Home Exercise Programs (HEPs).
  • Maintain timely and accurate documentation.
  • Collaborate with physicians, caregivers, and the interdisciplinary team to help ensure successful patient outcomes.

💼 Our Ideal HHC Guru

You may be our next Home Health Magic Maker if you have:

  • Degree from an accredited PT or PTA program.
  • Current Michigan PT or PTA license in good standing.
  • Home health experience preferred.
  • Strong clinical judgment and problem-solving skills.
  • Excellent communication and patient education skills.
  • Ability to work independently while remaining connected to the care team.
  • Reliable transportation, valid driver's license, and current auto insurance.
  • Familiarity with Medicare/CMS documentation preferred.
