About the Role

Description

The Medical Records Clerk is responsible for initiating health records charts and works closely with the front office personnel, providers, and staff. The Medical Records Clerk also maintains participants files and statistics, responds to requests for medical records and performs clerical duties.  

Requirements

  • Organize and maintain   health records by collecting information about participants: including test   results, diagnoses, exam results, recommended treatments, prior medical   history and other relevant data.
  • Maintains participants   files and retrieves files and documents for scheduled appointments: file   study patient charts, files participant data upon receipt of information, initiate records for new participants and creates computer index, prepare   file labels, maintains filing statistics and audits filing sequence.
  • Serve and protect High   Desert PACE by adhering to professional standards, High Desert PACE policies   and procedures, federal, state and local requirements.
  • Enhance health records   and the reputation of High Desert PACE by accepting ownership for   accomplishing new and different requests; exploring opportunities to add   value to job accomplishments. 
  • Serve as back up   coverage for front desk staff.
  • Keep supervisor   informed of problems or issues.
  • Retrieves participants   schedules from computer system; distribute lab reports to physicians and   materials to other departments, requests information from various departments   and responses to correspondence requests. 
  • Participate as a   member of the interdisciplinary team and in the initial, semi-annual,   unscheduled and annual assessments; attend morning updates and report changes   in participants baseline status to appropriate staff as needed.
  • Involved in the   development and implementation QAPI activities.
  • Attend and participate   in staff meetings, in-services, projects, and committees as assigned.
  • Adhere to and support   the center’s practices, procedures, and policies including assigned break   times and attendance.
  • Accept assigned duties   in a cooperative manner; and perform all other related duties as assigned.
  • Be flexible in   schedule of hours worked.
  • May require use of personal vehicle. 

Education & Training

  • Minimum of one (1) year of documented experience in working with a frail or elderly population.
  • High school diploma or equivalent
  • Current drivers license in the state of California
  • Medically cleared for communicable diseases and has all immunizations up-to-date before engaging in direct participant contact. 

Skills & Abilities

  • Proficient knowledge of computer skills, MS Office (Word, Excel, Access, PowerPoint and Outlook)
  • Knowledge of general office procedures, equipment and filing systems.
  • Ability to communicate effectively, both orally and in writing.
  • Ability to quickly learn department policies, procedures, goals and services.
  • Attention to detail and accuracy.
  • Ability to change priorities regularly.

  • Ability to access all areas of the center throughout the workday.
  • Ability to lift up to 35 pounds occasionally, 15 pounds frequently, and 7 pounds constantly; required to obtain assistance of another qualified employee when attempting to lift or transfer objects over 25 pounds.
  • Requires valid driver’s license.
