Overture Home Care is seeking an outgoing, relationship-driven Clinical Community Liaison (LVN) to help grow our presence in the DFW community while serving as a trusted clinical resource for clients, families, referral partners, and senior living communities.
This unique position combines nursing, community outreach, relationship management, and business development. You will build relationships with healthcare and senior care professionals, help connect families with the services they need, conduct clinical assessments, and play a direct role in growing Overture Home Care.
What You Will Do:
- Build and maintain strong relationships with hospitals, physicians, senior living communities, case managers, and other referral partners.
- Develop new referral sources while strengthening existing community partnerships.
- Represent Overture Home Care at networking events, conferences, community events, and outreach opportunities.
- Spend time within assigned senior living communities developing relationships with residents, families, staff, and leadership.
- Conduct clinical assessments, triage concerns, and recommend appropriate next steps within LVN scope of practice.
- Assist with pre-admission paperwork, assessments, and the admission process to help qualified referrals transition smoothly into care.
- Provide clinical education and support to clients, families, caregivers, and community partners.
- Conduct wellness clinics and services such as vital signs, blood sugar checks, and other appropriate LVN assessment activities.
- Maintain regular communication with clients, families, community partners, and Overture team members regarding client needs and condition changes.
- Track referrals, admissions, account activity, and business development efforts.
- Monitor referral trends and performance to identify opportunities for continued growth.
- Collaborate with Overture's clinical, operational, and leadership teams to provide an exceptional client experience.

Requirements
- Current, active, and unencumbered Texas Licensed Vocational Nurse (LVN) license required.
- A valid driver's license, working vehicle, and up-to-date auto insurance.
- At least 3 years of geriatric experience.
- Healthcare experience involving relationship development, community outreach, account development, or new business prospecting preferred.
- Strong interpersonal and communication skills.
- Comfortable developing new relationships and introducing Overture's services to potential referral partners.
- Ability to work independently, take initiative, and use sound judgment.
- Strong organizational and documentation skills.
- Comfortable balancing clinical responsibilities with business development goals.
- Ability to work effectively with older adults, families, healthcare professionals, and community partners.
- Flexibility to occasionally work outside normal business hours or on weekends when necessary.
