Heartline Staffing is recruiting a Family Nurse Practitioner for a full-time outpatient primary care position in Arcata, California. This is a panel-based primary care role caring for patients with complex medical needs in collaboration with an integrated care team.

POSITION OVERVIEW
• Full-Time 5x8 Schedule
• Outpatient Panel-Based Primary Care
• Care For Patients With Complex Medical Needs
• Six-Month Mentorship And Onboarding Program
• Integrated Care Team With 2 Medical Assistants And 0.7 RN Per Provider
• Access To Behavioral Health, Chiropractic And Specialty Telehealth Consult Services
• Rural Health / Underserved Population Setting

COMPENSATION
• $148,000-$163,000 Base Salary
• $12,400 Annual Access Bonus Paid Biweekly For Meeting 9-Patient Per Clinic Target
• Up To $44,000 Additional Annual Production Bonus Potential
• $25,000 Sign-On Plus Retention Bonuses For Partnership-Eligible Providers
• Alternative Sign-On And Retention Structure Available For Non-Partnership Track

RELOCATION & PROFESSIONAL SUPPORT
• Up To $10,000 Relocation Reimbursement
• Additional $10,000 Relocation Assistance For Qualifying Parent Or In-Law Relocation

• California Nurse Practitioner License Or Eligibility For California Nurse Practitioner Licensure REQUIRED
• Ability To Provide Outpatient Panel-Based Primary Care REQUIRED
• Ability To Care For Patients With Complex Medical Needs REQUIRED

Preferred
• 3+ Years Of Outpatient Experience Preferred
• Interest In Rural Health Or Serving Underserved Populations Preferred
• Spanish Language Skills Preferred - Differential Pay Available
