About the Role
Description
Bishop Gadsden is seeking a compassionate Registered Nurse Care Navigator to help older adults and their families navigate healthcare decisions, coordinate services, and maintain the highest quality of life.
This is a unique opportunity for an RN who enjoys relationship-building, advocacy, and care coordination in a non-bedside role.
What You'll Do:
- Guide residents and families through healthcare and aging-related decisions
- Coordinate appointments, services, and community resources
- Develop and support individualized care plans
- Monitor resident needs and communicate changes in condition or level of care
- Complete assessments and Long Term Care insurance documentation
- Partner with families and interdisciplinary teams to ensure excellent resident support
- Accompany residents to medical appointments as needed
Requirements
What You'll Bring:
- Active RN license in the state of SC
- 2+ years of nursing experience
- Knowledge of senior services and aging resources
- Ability to accurately and efficiently document care in a timely manner
- Strong communication, organization, and customer service skills
- Valid South Carolina driver's license and clean driving record for the past 3 years
