School-Based Case Manager II - Bilingual

Summary:

Palmer is seeking an experienced, compassionate, and motivated Bi-lingual Case Manager II to provide high-quality, client centered services to adolescents, caregivers, and adults experiencing co-occurring mental health and substance use disorders in an outpatient setting. The ideal candidate will possess strong case management, communication, and organizational skills, along with working knowledge of addiction, recovery, and community resources. 

The Case Manager II will empower consumers to identify and achieve self-determined recovery goals by providing comprehensive case management services, including assessment of biopsychosocial needs, care coordination, referrals, linkage to community resources, advocacy, monitoring, and ongoing support throughout treatment. The position emphasizes a collaborative, strength-based approach that prompts recovery, wellness, and improved quality of life.

The successful candidate will demonstrate professionalism, culture competence, trauma informed care, and a commitment to supporting individuals and families throughout the recovery process while maintaining compliance with OKMHSAS, CCBHC, and agency standards.  

Responsibilities:

• Verify that the NPI (National Provider Identifier) remains active and in good standing with NPPES; renew as necessary.
• Confirm that the Provider Number for Medicaid billing is valid and current with OHCA; renew as required.
• Align all work practices with Palmer’s mission, goals, and core values.
• Maintain strict confidentiality in accordance with Palmer policies, State of Oklahoma regulations, and all applicable federal laws.
• Complete Relias training (The Use of Telehealth in Clinical Practice) and sign the Telehealth (ICT) Policy and Procedure Acknowledgement prior to using telehealth with consumers.
• If providing school-based services, complete Botvin curriculum training, participate in start-of-the-school-year meetings, complete weekly tracking reports for Palmer, and submit monthly reports to Tulsa Public Schools by the 5th of each month for each contracted site.
• Stay informed of updates and changes to HIPAA, ODMHSAS, CCBHC, 42 CFR, and CARF standards.
• Comply with all ODMHSAS/CCBHC Case Management requirements and standards.
• Ensure that all record documentation is complete, accurate, and accessible only to authorized personnel.
• Assist with internal chart audits and external accreditation reviews as needed for ODMHSAS and CARF.
• Communicate promptly with the Outpatient Administrative & Clinical Director regarding staff concerns, confidentiality breaches, and critical incident reports.
• Demonstrate professionalism and ethical conduct at all times, including behavior on social media platforms.
• Follow Palmer’s Employee Handbook policies regarding dress code, workplace behavior, and conduct expectations.
• Complete all required in-service trainings assigned through Relias by HR and/or the Outpatient Administrative & Clinical Director.
• Actively participate in monthly all-staff meetings and case management staffing.
• Develop and maintain Family Care Plans for pregnant clients and families requiring additional support.
• Provide consumers with appropriate linkage, referrals, follow-up, monitoring, and advocacy for community-based resources.
• Initiate intakes in both EMR systems: Cantata Health Solutions for ODMHSAS clients and GoHealth for Medicaid clients entering into a CCBHC.
• Complete 12 hours of continuing education (CEUs), including 3 hours in Ethics and 9 hours in Co-Occurring, and pay the $15.00 recertification fee to the State of Oklahoma by June 1 of each fiscal year to maintain certification.
• Perform additional duties as assigned by the Outpatient Administrative & Clinical Director in support of the outpatient department.

Requirements

Requirements:

 • Current Behavioral Case Manager II certification through ODMHSAS.
• Current Oklahoma Driver’s License and acceptable Motor Vehicle Record (MVR).
• ASAM PPC-2R training completed by an ODMHSAS-certified trainer within the past five years.
• Ability to develop and maintain collaborative relationships with a variety of referral sources.
• Excellent verbal and written communication skills.
• Advanced interpersonal skills.
• Strong computer skills, including proficiency in Microsoft Word, Excel, and Outlook.
• Bilingual proficiency in English and Spanish preferred.
• Previous experience working in addiction recovery services preferred.
• Previous experience working with adults and adolescents preferred.

Physical Requirements:

• Ability to lift 30 pounds, sit for up to 8 hours/day, view computer screen for up to 8 hours/day.
• Job requires twisting, lifting, standing, sitting, walking, stretching, kneeling, bending, and squatting.
