Position Summary

The Receptionist/Transportation & Appointment Coordinator is responsible for providing professional front-desk and telephone coverage while coordinating resident medical appointments and transportation services. This position serves as a central communication point between residents, families, nursing staff, physicians, hospitals, transportation providers, visitors, vendors, and other departments.


Essential Duties & Responsibilities

  1. Answer incoming telephone and intercom calls, route calls appropriately, and make overhead announcements as necessary. 
  2. Greet and assist residents, families, visitors, vendors, physicians, and agency personnel in a courteous and professional manner. 
  3. Coordinate resident medical appointments, diagnostic testing, specialty consultations, and other outside healthcare services. 
  4. Schedule and coordinate transportation to and from resident appointments, including communication with drivers and transportation providers. 
  5. Coordinate resident transportation and required arrangements for hospital transfers and returns. 
  6. Prepare appointment paperwork, medical records, forms, and other documentation required to accompany residents to appointments. 
  7. Upon residents' return, ensure appointment information, progress notes, and new orders are routed to the appropriate licensed nursing staff. 
  8. Coordinate in-house appointments with physicians, podiatry, dental, hearing, vision, and other healthcare providers. 
  9. Maintain accurate census information, including admissions, discharges, room changes, transfers, and deaths. 
  10. Assist nursing staff with communication, routing slips, dietary requests, work orders, and other administrative needs. 
  11. Monitor the call panel and assist with emergency communication and overhead paging in accordance with facility emergency and disaster procedures. 
  12. Maintain appropriate visitor sign-in processes and resident sign-out procedures. 
  13. Assist residents with telephone calls, messages, and other reasonable needs. 
  14. Coordinate and support relief receptionist coverage and provide appropriate orientation regarding receptionist responsibilities. 
  15. Maintain a professional, organized, and welcoming reception area. 
  16. Communicate effectively with hospitals, physicians' offices, transportation providers, facility departments, residents, and families. 
  17. Participate in facility committees, disaster planning activities, meetings, and other assignments as directed. 
  18. Maintain confidentiality of resident information in accordance with facility policies and applicable privacy requirements. 
  19. Demonstrate professionalism, courtesy, compassion, and appropriate customer service at all times. 

Requirements

  • High school diploma or equivalent. 
  • Ability to speak, read, and write English effectively. 
  • Strong telephone, communication, organizational, and customer-service skills. 
  • Ability to work effectively with elderly and cognitively impaired residents. 
  • Previous long-term care or healthcare experience preferred. 
  • Ability to prioritize multiple responsibilities and work effectively with frequent interruptions. 
  • Ability to sit, stand, walk, and travel between buildings or departments as required.
