Job Summary

The RN–Rehabilitation provides direct and individualized nursing care to patients in a rehabilitation setting, utilizing the nursing process to assess, plan, implement, and evaluate patient care. This RN role focuses on supporting patients with physical, functional, or neurological impairments as they recover and regain independence. The RN collaborates with a multidisciplinary team to facilitate optimal patient outcomes through restorative care and education.

Essential Functions

  • Delivers comprehensive nursing care using the nursing process (assessment, planning, implementation, and evaluation)
  • Monitors, documents, and communicates patient conditions and progress
  • Administers medications, treatments, and therapies as prescribed
  • Assists patients with activities of daily living (ADLs), mobility, and rehabilitation exercises
  • Coordinates care and collaborates with therapy and medical staff to meet patient goals
  • Provides patient and family education related to rehabilitation, safety, and wellness
  • Promotes a safe environment, follows infection control policies, and uses PPE appropriately
  • Participates in patient discharge planning and ensures continuity of care
  • Responds to emergencies and acts within the RN scope of practice and policy
  • Supports hospital mission, goals, and performance improvement initiatives

Knowledge/Skills/Abilities/Expectations

  • Knowledge of rehabilitation nursing principles, functional recovery, and chronic condition management
  • Critical thinking and clinical judgment in patient assessment and care planning
  • Ability to adapt to diverse patient needs and age-specific considerations
  • Effective communication with patients, families, and interdisciplinary team members
  • Strong documentation and computer skills
  • Ability to work collaboratively and promote a positive team environment
  • Capable of physical activity including lifting, moving patients, and standing/walking for extended periods 


Education

  • Graduate of an accredited nursing program, required
  • Bachelor of Science in Nursing (BSN), preferred

Licenses/Certifications

  • Current Registered Nurse license in the state of practice or compact license
  • Basic Life Support (BLS) required  

Experience

  • Clinical nursing experience preferred
  • Previous experience in rehabilitation or med-surg setting preferred
