Position Summary:
- Manage and ensure the effective delivery of evidence-based caregiver support services, promoting high-quality, person-centered care that supports clients and caregivers.
- Lead staff and service operations by setting priorities, managing resources, ensuring compliance, and driving quality improvement to deliver coordinated, person-centered services for older adults and caregivers.
Essential Duties and Responsibilities
The following description is a general representation of the key duties and responsibilities of the position.
- Direct the day-to-day operations of caregiver support programs, including WeCare, SHARE, Caregiver Relief Programs, and other evidence-based caregiver support services, ensuring high-quality, person-centered service delivery that meets organizational, contractual, payer, and regulatory requirements.
- Identify, pursue, and support new funding opportunities, including grants, contracts, and payer partnerships, to advance program growth and sustainability.
- Develop and maintain relationships with healthcare providers, community organizations, referral partners, and other stakeholders to increase awareness of caregiver support services.
- Conduct outreach, presentations, and educational activities to promote caregiver programs, generate referrals, and connect caregivers with available resources.
- Collaborate with Institutional Advancement to develop and execute strategic marketing plans, maintain relevant content, and create high-impact presentations and publications for current and prospective partners.
- Provide operational guidance and consultation to staff regarding caregiver support services, documentation standards, community resources, care consultation models, and complex client situations to ensure consistent, high-quality service delivery.
- Oversee program documentation, billing, reporting, and record management to ensure accuracy, completeness, compliance, and fulfillment of organizational, contractual, and funding requirements.
- Lead the implementation and continuous improvement of evidence-based caregiver support models, organizational care consultation protocols, workflows, and service standards to promote consistency and service excellence.
- Collaborate across departments to support program operations, organizational planning, quality improvement initiatives, interdisciplinary projects, and other efforts that advance the organization's mission and caregiver service objectives.
- Maintain current knowledge of applicable laws, regulations, ethical standards, payer requirements, organizational policies, community resources, and evidence-based caregiver support practices.
- Ensure staff compliance with applicable regulatory, contractual, documentation, confidentiality, HIPAA, and adult protective services reporting requirements.

Requirements
Specific knowledge, training or skills required to perform the duties of this position including specific courses, training programs or required certifications. The requirements listed below are representative of the knowledge, skills and abilities required for the position.
- Master’s degree in social work, healthcare administration, human services, gerontology, public health, or a related field required.
- Current and unrestricted Ohio licensure, where applicable.
- Strong verbal, written, oral and electronic communication skills.
- Demonstrated ability to effectively present and communicate with diverse audiences, including community groups, healthcare providers, referral partners, caregivers, and other stakeholders, about caregiver support programs and services.
- Strong financial, analytical, organizational, and problem-solving skills, including the ability to use data and financial information to inform decisions.
- Able to work independently on most clinical and administrative matters, while seeking consultation and supervision as appropriate.
- Excellent communication, presentation, relationship-building, and stakeholder engagement skills.
- Strong knowledge of caregiver support, aging services, community-based services, and the needs of older adults and family caregivers.
- Excellent organizational skills and ability to handle multiple tasks and prioritize work.
- Demonstrated proficiency in use of computer software (e.g. Microsoft Outlook and Office), database management and/or financial software support.
- An insured car and valid Ohio driver’s license are required.
Previous Experience that is necessary to qualify for this position:
- Minimum of 5 years of progressive experience in aging services, caregiver support, healthcare, human services, or a related field, including significant experience overseeing programs and services.
- Previous experience managing contracts or partnerships with third-party payers, local healthcare systems or community-based organizations or associations.

This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
- While performing the duties of this job, the employee is regularly required to, stand, sit; talk, hear, reach, stoop, kneel, and use hands and fingers to operate a computer, keyboard and telephone.
- Specific vision abilities required by this job include close vision requirements due to computer work.
- Light to moderate lifting is required, up to 25 pounds.
- Ability to uphold the stress of traveling.
- Regular, predictable attendance is required.
- Moderate noise (i.e. business office with computers, phone, and printers, light traffic).
- Ability to work in a confined area.
- Ability to sit at a computer terminal for an extended period of time.
