Services include: Medical Dental Behavioral Health Pharmacy HIV Prevention, Care & Treatment Homeless Support Services Medication-Assisted Treatment for Substance Use Disorder This health center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n).
About the Role


As the Homeless Support Services Program Manager, you will lead and coordinate grant-funded programs that provide case management and housing services for individuals experiencing homelessness. This role oversees daily operations, leads assigned staff (10 - 15), and ensures high-quality services through interdisciplinary collaboration, compliance monitoring, reporting, and quality assurance.  

This position typically works at PHC University Campus in Des Moines but travels to other off-site locations as needed. This position is typically scheduled to work Monday - Friday 8 AM - 5 PM. Our team members are occasionally needed to work an evening or weekend event. Mileage reimbursement is available for required travel. 


What You Will Do

  1. Collaborates with leadership colleagues to oversee the day-to-day operations of the Homeless Support Services Department (HSS). 

  2. Leads and develops program staff by establishing clear expectations, providing training and resources, overseeing hiring and onboarding, managing performance evaluations and competency assessments, maintaining position descriptions, and providing ongoing coaching, guidance, and accountability. 

  3. Monitors grant revenues and expenditures, develops and manages the program budget, and makes fiscally responsible decisions. Attends monthly budget meetings, reviews billing statements, and approves daily expenditures to ensure compliance with established budgetary limits.  

  4. Develops and maintains relationships with community partners, other PHC departments, and funding and oversight agencies through proactive communication and active participation in applicable groups and meetings. Conducts public relations and client liaison activities, including conflict resolution and complaint disposition. Educates internal staff and external agencies about PHC and HSS services.

  5. Participates in writing grant applications and reports for the department.

  6. Ensures compliance with grant, regulatory, and PHC requirements by conducting quality assurance reviews of case files, documentation, and data; verifying the accuracy and appropriateness of case notes; and ensuring adherence to applicable policies and procedures. 

  7. Develops and facilitates performance improvement initiatives to ensure effective and efficient workflows. 

  8. Supports and empowers clients throughout the Continuum of Care including accessing other PHC or homeless support services and addressing barriers to care.


Qualifications You Need

Required

  • Driver’s license. 

  • Bachelor’s degree in health care, human services, social work, psychology, counseling, mental health, or a related field. Equivalent experience may be considered in lieu of the required education if allowed by the funding agency.

  • A minimum of two years of experience working with local housing or community resources. 

  • Must be obtained within 90 days of employment: 

    • Mandatory Reporter Training for Child Abuse and Dependent Adult Abuse recognized by the State of Iowa.

    • State of Iowa Chauffeur’s license.

  • Leadership and management skills with ability to effectively coach, guide, and engage for professional development and goal achievement.

  • Basic computer proficiency, including use of email, word processing, and spreadsheet applications.

  • Excellent interpersonal, teamwork, and collaboration skills.

  • Effective written and verbal communication and listening skills.

  • Excellent customer service skills with a commitment to service excellence.

  • Demonstrates initiative with ability to work independently with minimal supervision.

  • Strong organization, prioritization, and time management skills.

  • Detail orientation with a high degree of accuracy.  

  • Effective problem-solving, analytical, and critical thinking skills.

  • Demonstrates professionalism, interacts with others calmly and empathically, and de-escalates difficult situations.


Preferred

  • Fluency in spoken English and Spanish or a language commonly spoken by the department’s clients, with the ability to communicate clearly and accurately in both languages. 

  • Experience in a community health center or working with grant-funded programs.

  • Proficiency using Homeless Management Information System.

Your related experience and skills determine your base pay. The typical hiring range for this position is $47,120 - 58,900 annually. Candidates with extensive related experience may be considered for a higher pay rate up to the pay range maximum of $70,680. In addition to base pay, PHC offers a comprehensive benefits package, including:
