Hometown Home Health provides compassionate, high-quality care in the comfort of our patients’ homes. As a full-service home health agency, we offer skilled medical and clinical services designed to support each patient’s individual needs, independence, and quality of life.


What We’re Searching For

Hometown Home Health is seeking a licensed Speech-Language Pathologist to provide patient-centered care in the Jacksonville area. We’re looking for someone who is comfortable working independently in a home health setting and is passionate about helping patients improve their communication and swallowing abilities.


What You’ll Do

  • Provide speech-language pathology evaluations, assessments, and treatment in patients’ homes
  • Assess patients’ communication, cognitive, and swallowing abilities
  • Develop individualized treatment plans in coordination with the patient’s physician and care team
  • Provide therapy designed to improve communication, speech, language, cognition, and swallowing as appropriate
  • Educate patients and caregivers on treatment plans, strategies, and home exercises
  • Monitor patient progress and adjust treatment plans as needed
  • Complete accurate and timely documentation, including visit notes and progress reports
  • Collaborate with physicians, nurses, therapists, and other members of the interdisciplinary care team

What You Need

  • Master’s or Doctoral degree in Speech-Language Pathology
  • Current Florida Speech-Language Pathologist license
  • Current CPR certification; online-only certification is not accepted
  • At least one year of relevant clinical experience
  • ASHA Certificate of Clinical Competence (CCC-SLP) preferred
  • Strong communication, organization, and patient-care skills
  • Ability to travel to patients’ homes throughout the assigned service area
