About the Role
Description
Community Services Direct Support Professional
Location: Norwich, NY
Schedule: Part-time, up to 29 hours per week
Pay: $17.50- $18.75

The Community Services Direct Support Professional is responsible for providing direct care through implementation of Habilitation Plans. Specific duties and responsibilities include, but are not limited to:
- Provide supervision and all direct care required to maintain safety and comfort of the person receiving services
- Transport or accompany people receiving services on trips to community outings, appointments, or recreation activities for the purpose of community integration and exploration
- Development and planning of inclusive community activities
- Establish and maintain positive relationships with area businesses
- Assist people receiving services with completing daily or routine domestic or personal care tasks
- Foster and promote skills leading to independent living
- Assist with the development of Habilitation Plans and following these plans for training and assistance
- Provide training and assist people receiving services to develop, maintain and/or improve desired individual goals, living skills and personal needs as assigned or required
- Provide supervision to people supported during program hours and during community outings as to ensure their health and safety
- Complete required documentation in conformance with rules, regulations, policies and procedures, and billing practices
Requirements
- High School Diploma/GED
- Preferred: 2 years of experience working with the individuals with disabilities Or A combination of education and experience that in the opinion of the reviewing agency, exceeds the above standards of the program
- Possess a valid driver’s license acceptable to the agency’s insurance carrier.
