About the Role
Description
- Patient Assessment & Care Planning
Evaluate health conditions, monitor symptoms, and develop/implement personalized care plans in coordination with physicians and other healthcare providers - Clinical Procedures
Administer prescribed medications, perform wound care, manage IV therapy, change dressings, and monitor vital signs - Health Education
Teach patients and families proper home care techniques, medication management, and disease prevention strategies - Coordination of Care
Collaborate with doctors, physical/occupational therapists, and other professionals to ensure continuity of care . - Documentation
Maintain accurate, timely medical records and update providers on patient progress - Support & Comfort
Provide emotional support, ensure a safe and clean home environment, and assist with daily activities when needed i - Specialized Care
May deliver palliative care, post-surgical wound management, or chronic disease monitoring depending on the care plan
