Care Partner - Blacktown and surrounds (Full-Time Permanent)

Do you have experience supporting older Australians and a passion for helping people navigate their aged care journey?

As a Care Partner, you will work closely with clients receiving government-funded aged care services in the Blacktown region. You'll build meaningful relationships with clients, assess their needs, coordinate services and advocate for positive outcomes that help them remain safe, independent and connected in their own homes.

This is a full-time permanent position, combining community-based client visits with the flexibility to work autonomously from home.

What we're looking for

• Experience in community-based case management will be highly regarded.
• Experience supporting clients receiving government-funded aged care services is advantageous.
• Strong communication and relationship-building skills.
• Demonstrated empathy, compassion and a client-centred approach.
• Effective problem-solving and critical thinking skills.
• Well-developed organisational and time management skills.
• Ability to work collaboratively across multidisciplinary teams.
• Advocacy skills to support positive client outcomes.
• Current driver's licence and a reliable vehicle with appropriate insurance.

The role

• Manage a caseload of older Australians receiving government-funded aged care services in their homes.
• Conduct client assessments and develop personalised Care Plans.
• Monitor and manage client budgets in line with government program requirements.
• Coordinate services to ensure clients receive the right support at the right time.
• Support new clients through onboarding and ongoing service delivery.
• Build strong relationships with clients, families and key stakeholders.
• Work closely with clinical, scheduling and service delivery teams to deliver coordinated care.
• Advocate for client needs and support positive health and wellbeing outcomes.
