Primary Health Care (PHC) was founded in 1981 by Dr. Bery Engebretsen in Des Moines, IA. Our mission has remained unchanged since that time, to provide healthcare and supportive services to all, regardless of insurance, immigration status, or ability to pay.
Based on the needs of the communities we serve, PHC offers a spectrum of medical and dental services including family practice, behavioral health, HIV care and services, and pharmacy. PHC’s Homeless Support Services is the entry point for serving people experiencing homelessness in Polk County. Enabling services are available to help patients with benefits enrollment, case management, transportation, translation, and patient education. We currently have locations in Ames, Des Moines, & Marshalltown.
As a Case Manager Team Lead for Homeless Support Services, you will be responsible for leading assigned case management staff. Provides case management to program clients and/or patients, advocating for clients with other agencies, and assisting clients to access services, according to grant guidelines. Responsible for tracking and reporting grant data. Facilitates weekly meetings for the grant team. Contact may occur in nontraditional settings.
What You Will Do
- Coordinates case management duties for assigned area(s), ensuring key tasks are completed.
- Leads team meetings for the grant team and attends all grant related activities.
- Provides initial consultation for escalated client situations.
- Completes intakes and ongoing assessment with clients to identify needs, develop goals, and access resources and services. May assist clients with the application process for entitlement programs.
- Monitors and documents client contacts, needs, goals, progress, and significant occurrences, according to grant guidelines and PHC policies and procedures.
- Enters data into applicable data systems accurately and timely, maintaining productivity standards in accordance with policies and procedures.
- Reviews entered data each month to ensure accuracy and timeliness. Collaborates with data system provider and grantor to ensure that reporting is completed on time.
- Collaborates with Program Director and Grant Accountant monthly to ensure that all grant spending is up to date and within eligible means.
- Facilitates coordination with other PHC services, including medical, dental, mental health, and substance use, as well as referrals for services with external agencies, including emergency and crisis services.
- Advocates for clients with community agencies, schools, significant others and other appropriate agencies.
Qualifications You Need to Bring
Required:
Bachelor’s degree or equivalent combination of education and experience (if allowed by granting agency).
A minimum of 2 years of experience in human services.
Supervisory skills with ability to effectively provide direction and feedback.
Excellent customer service skills.
Excellent verbal and written communication skills along with effective listening skills.
Effective organization and time management skills with ability to work independently.
Leadership skills with demonstrated ability to gather a team to work toward common goals.
Proficiency using Microsoft Office applications and internet-based applications; willingness and ability to learn different software and databases.
Strong interpersonal skills with ability to establish and maintain effective working relationships along with ability to de-escalate difficult situations.
Detail oriented with high accuracy.
Demonstrates professionalism with the ability to interact with others in an empathetic manner.
Problem solving and critical thinking skills.
Licenses & Certifications: *Valid Iowa Chauffeur’s License (Class D); *Current Mandatory Reporter Training: Child Abuse and Dependent Adult Abuse recognized by the State of Iowa.
* Must be obtained within the Introductory Period if not current.
Preferred:
Bachelor’s degree in health care, psychology, counseling, mental health, or human services.
Work experience with vulnerable populations, including people experiencing homelessness, in a housing-related role, or in a community health center..
Working knowledge of local community resources, mental illness, and/or chemical dependency.
Previous military service.
Ability to effectively use the Homeless Management Information System (HMIS).
