OLDER ADULT HEALTH NAVIGATOR JOB SUMMARY


The Older Adult Health Navigator plays a vital role in engaging and supporting older adult patients and their caregivers, through a person-centered, culturally responsive approach. The Older Adult Health Navigator builds trusting relationships with patients and serves as a bridge between the patient, their medical and other providers, and their caregivers. This position connects patients to resources, and guides the older adult and their through the health care system. 



OLDER ADULT HEALTH NAVIGATOR ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Conducts internal outreach activities to identify older adults and their caregivers. 
  • Works closely with medical, dental and behavioral staff to identify potential patients 
  • Facilitates navigation throughout the CCHC system to ensure coordinated care and follow up
  • Utilizes EHR to coordinate services and input data for reporting. 
  • Utilizes resource opportunities for older adults and caregivers to enhance quality of life and continuity of care
  • Suggests new services, based on assessment of older adult and caregiver needs 
  • Supports the development and implementation of new services for older adults and caregivers and encourages participation in these services 


Requirements

OLDER ADULT HEALTH NAVIGATOR EDUCATION, TRAINING AND EXPERIENCE

  • Associate or Bachelor’s degree required; additional certification or training as a Community Health Worker, peer advocate, or related paraprofessional role may be considered in lieu of degree
  • Previous experience in a healthcare, social services, or community-based setting preferred.
  • Strong desire to work with older adults in an empathic, person centered, respectful and non judgmental manner
  • Demonstrated commitment to community engagement and supporting underserved populations.
  • Ability to model positive health behaviors and support both individual and community capacity-building. Strong communication, interpersonal, and advocacy skills to effectively engage with members, families, healthcare providers, and community organizations.


OLDER ADULT HEALTH NAVIGATOR KNOWLEDGE, SKILLS AND ABILITIES

  • Ability to build rapport, trust, and supportive relationships with patients from diverse cultural and socioeconomic backgrounds.
  • Skilled in communication and active listening
  • Familiarity with local community resources, public assistance programs, and social service systems.
  • Ability to work independently and collaboratively as part of an interdisciplinary team.
  • Strong organizational skills, with the ability to manage multiple patients and tasks.
  • Comfort with learning documentation, data entry, and use of electronic health records or case management systems.
  • Commitment to cultural humility and patient-centered engagement.
  • Ability to find solutions when barriers are identified.
  • Good documentation skills.
  • Ability to multi-task and prioritize when needed.
  • Ability to independently seek out resources and work collaboratively.
  • Ability to read, understand and follow oral and written instructions.
  • Demonstrates a willingness and ability to work under supervision.
  • Ability to develop and maintain good working relationships with staff.
  • Ability to use computer and learn new software programs.
  • Excellent interpersonal skills reflecting clarity and diplomacy and the ability to communicate accurately and effectively with all levels of staff and management.
  • Possesses ability to communicate effectively, both verbally and in writing.
  • Demonstrate flexibility, enthusiasm, and willingness to cooperate while working with others in multi-disciplinary teams.
  • Performs other related duties as assigned.

OLDER ADULT HEALTH NAVIGATOR PAY RATE: $24/hr
