Pipelines to Possibility (P2P) is a Tier 1 plumbing certification program operated by VOAMASS in conjunction with the Essex County Sheriff’s Department .
As Case Manager, you’ll have a dynamic and critical role supporting program participants both pre- and post-release to find and maintain employment. The Case Manager will conduct intake and assessments, work with participants to develop Individualized Development Plans, and provide ongoing advocacy, education, referrals, and coordination of community support services to help reduce barriers to successful reentry, with a specific focus on promoting sustainable employment outcomes. The Case Manager will work closely with the Program Director, Essex County Sheriff’s Department staff, MassHire, the Plumbing Instructor, employers, and other partners to identify and refer eligible participants, coordinate services, and support successful transition from training to employment in climate-critical jobs.
Schedule: This position includes work within the Essex County Pre-Release and Reentry Center, VOAMASS offices, community settings, and employer locations. Regular travel between program sites, community providers, employers, and other locations may be required. Your schedule may require flexibility to accommodate participant release dates, employer meetings, program activities, and participant needs.
Case Manager Key Responsibilities:
- Coordinate with Program Director and ECSD staff to identify, screen, refer, and enroll eligible participants in P2P.
- Conduct participant intake, assessments, and strengths-based evaluation of employment goals, work history, skills, and barriers to successful participation and employment.
- Develop and maintain Individualized Development Plans (IDPs) with participants and update plans as needs, goals, release dates, and employment circumstances change.
- Provide individualized case management and participant support before and after release, including regular one-on-one sessions in the community post-release.
- Identify barriers to training, reentry, employment, and retention, including transportation, identification/work documentation, housing instability, behavioral health needs, digital access, and other support needs; coordinate referrals and follow-up as appropriate.
- Educate and advocate for participants regarding available community resources and support services and help ensure successful connection to referred services.
- Work closely with the Plumbing Instructor, Program Director, MassHire staff, ECSD staff, and other partners to coordinate participant progress, workforce readiness, and employment planning.
- Support participants with employment readiness activities, including resumes, interview preparation, job search strategies, career planning, and connections to appropriate training or credential opportunities.
- Support employer outreach and job development activities as needed, including identifying potential employers, coordinating employer engagement, and helping connect qualified participants to employment opportunities.
- Provide post-placement support to participants and coordinate with employers and partners, as appropriate, to address barriers that may affect employment retention.
- Maintain timely and accurate case notes, participant records, service documentation, referrals, employment outcomes, and other required program data in CareLogic electronic health record and/or designated program tracking systems.
- Participate in case conferences, weekly supervision, program meetings, required trainings, and collaborative planning with VOAMASS and partner agencies.
- Assist the Program Director and Plumbing Instructor with program administration, participant coordination, employer outreach, and other program activities as needed.
- Perform other duties related to the successful implementation of the program as assigned.
The Case Manager role is about making a difference in the lives of the people we serve. As Case Manager, you’ll bring the following qualifications:
- Bachelor’s degree in social work, psychology, education, rehabilitative counseling, human services, or a related field preferred; relevant experience may be considered.
- Experience providing case management, reentry, workforce development, employment, or related human services preferred.
- Experience working with justice-involved individuals and individuals experiencing barriers to employment preferred.
- Strong communication, organization, documentation, and relationship-building skills.
- Proficiency in Microsoft Office, including Word, Excel, Outlook, and Teams.
- Ability to work collaboratively with correctional, workforce, training, employer, and community partners.
- This position requires independent travel between worksites and community locations; valid driver’s license and insurance coverage on personal vehicle required.
- Ability to pass Essex County Sheriff’s Department security clearance requirements.
