Responsibilities:
- Provide structured, community-based adult day treatment services to individuals with Severe and Disabling Mental Illness (SDMI) who experience significant functional impairment.
- Deliver individualized and group-based therapeutic and psychosocial rehabilitation interventions consistent with each member’s comprehensive assessment and individualized treatment plan (ITP).
- Assist members in developing and strengthening independent living, emotional regulation, crisis-management, socialization, interpersonal, pre-vocational, vocational, and community-functioning skills.
- Implement treatment activities that promote appropriate behavior and progressively greater independence in daily and community functioning.
- Provide Community-Based Psychiatric Rehabilitation and Support (CBPRS) and group therapy services within the bundled day treatment program, as clinically appropriate and authorized.
- Ensure interventions address the member’s diagnosed mental health needs and are reasonably expected to produce measurable outcomes related to the member’s SDMI.
- Observe, monitor, and document each member’s participation, response to interventions, functional progress, behavioral presentation, and changes in condition.
- Maintain accurate and timely progress notes in the member’s clinical record and whenever a significant change in condition occurs, or more frequently as required by organizational policy.
- Participate in the development, implementation, monitoring, and 90-day review of individualized treatment plans, ensuring medically necessary day treatment interventions are clearly identified.
- Provide services for the authorized duration of two to three hours per member per day unless an exception is granted by the appropriate regulatory authority.
- Maintain a member-to-staff ratio of no more than 10 members to one staff member and immediately communicate staffing or safety concerns to the program supervisor.
- Maintain active supervision of members during programming and accurately account for periods when members are not present or participating.
- Ensure recreational, social, and educational activities are clinically structured, supervised, connected to the member’s treatment needs, and designed to support measurable rehabilitation outcomes.
- Recognize and respond appropriately to behavioral health crises using approved therapeutic de-escalation (HELP), crisis-intervention, and safety procedures, also performs basic First Aid and/or CPR when needed.
- Collaborate with the program supervisor, licensed clinical mental health professionals, treatment team members, families, and community providers to promote continuity of care and successful community functioning.
- Report significant changes in a member’s symptoms, functioning, behavior, safety, or service needs promptly to the program supervisor or licensed clinician.
- Meet established monthly service and productivity benchmarks
- Complete at least six contact hours of annual training related to adult mental illness and treatment.
- Complete initial and annual training in therapeutic crisis de-escalation, including approved physical and nonphysical methods of maintaining member and staff safety.
- Follow all applicable federal and state regulations, Medicaid requirements, licensing standards, confidentiality requirements, agency policies, and professional and ethical standards.
- Perform other job-related duties as assigned to support the effective and compliant operation of the adult day treatment program
- Attends and participates in mandatory training, in-services, staff meetings, and Corporate Committees assigned.
- Utilizes and accurately records data using AWARE’s Electronic Medical Records System as assigned, outlined, and trained.

Requirements
Talents, skills, and abilities:
- Ability to implement individual treatment plans under AWARE's Unconditional Care Philosophy, incorporating trauma-informed behavioral techniques, interventions, and skill-building strategies while understanding and honoring the emotional and behavioral needs of individuals with developmental disabilities.
- Basic computer literacy, including navigation, recording, and use of performance-based software and electronic medical records systems.
- Strong interpersonal, oral, and written communication skills to collaborate as a team member, make accurate observations, report them e?ectively, learn supportive care methods, and follow instructions—all while building trust and promoting empowerment.
- A High School Diploma or HSE is required, along with past employment experience.
- Must have or be willing to obtain a valid Montana Driver's License, with an acceptable driving record. Must pass a criminal and protective services background check.
