Outpatient mental health and substance use services for adults and children

Summary:


The Recovery Support Specialist provides skill building, engagement, and care coordination for patients in recovery at our Primary Care and ensuring care is effectively coordinated through InterCommunity’s recovery system of care.


Essential Duties & Responsibilities:

  • Complete continuing education credits to maintain Recovery Support Specialist Certificate annually.
  • Share knowledge and lived experience as a person in recovery.
  • Active participation on a transdisciplinary team including daily meetings to review services of clients served.
  • Assist in linking each client to the necessary clinical, medical, social, educational, rehabilitative, vocational and/or other services.
  • Collaborate within the MAT and care teams and the clients to identify required services by assessing the overall needs of each client.
  • Support client recovery by participating in individual treatment sessions within InterCommunity or outside providers if necessary.
  • Complete Wellness Management and Recovery interventions (WRAP, Peer Counselor Trainings).
  • Complete all required documentation related to treatment in accordance with established procedures including but not limited to court letters, gathering medical records, copying, faxing, scanning, collateral contact, and appointment scheduling.
  • Assertive engagement with clients and support ongoing attendance by completing all tracking documents andreports to support engagement.
  • Create and facilitate groups based on client identified needs (both office and community based).
  • Provide assistance during the assessment of individuals in crisis and intoxicated clients by helping to determine the need for crisis involvement or detox and facilitate the admission to detox facility.
  • Provide education, support and consultation to MAT team, clients and the individual’s families or their support system which is directed exclusively to the well being and benefit of the individual, if needed.
  • Performs related duties as required.

Schedule:

Sunday - Thursday, 7:00 AM - 3:30 PM



Requirements

Education &/Or Experience:


High School Diploma is required. Valid driver’s license required; Graduates of Advocacy Unlimited program are highly to apply encouraged. CPR and First-Aid certification is to be obtained within 6 months of hire. Prior experience working with people with severe and prolonged mental illnesses and/or substance abuse disorder is required.
