Housing & Healthcare Specialist

Description
This position plays an important role in helping individuals and families overcome barriers to stable housing, healthcare, and essential community resources.
Under the supervision of the Housing & Health Navigation Supervisor, the Specialist provides individualized case management, housing assistance, healthcare navigation, advocacy, and ongoing support to vulnerable and underserved populations.The ideal candidate is passionate about community service, committed to providing trauma-informed care, and comfortable working both independently and collaboratively in office and community settings.
Key Responsibilities
- Conduct participant intakes, assessments, and eligibility determinations for housing and healthcare services
- Develop individualized housing and healthcare plans to address participant needs and barriers
- Provide ongoing case management, advocacy, follow-up, and referrals to community resources
- Assist participants with securing and maintaining stable housing and accessing healthcare and behavioral health services
- Coordinate with landlords, healthcare providers, community organizations, and other service partners
- Conduct home visits, participant safety check-ins, outreach activities, and community meetings
- Facilitate workshops and participant education related to housing, employment, benefits, and life skills
- Maintain accurate and timely case notes, participant records, and program documentation
- Complete required data entry and reporting in HMIS and internal systems, including ECM and CCAH requirements
- Support program operations, participant engagement, and housing-related activities
- Follow program workflows, policies, and procedures to maintain high-quality service delivery
- Assist in tracking program metric, data entry, and reporting requirements for funders, including ECM for CCAH
Requirements
Required:
- High school diploma or GED
- Strong organizational, communication, and problem-solving skills
- Ability to manage multiple priorities and work independently
- Proficiency in documentation, data entry, and reporting
- Valid driver's license and reliable transportation
- Successful background clearance
- Commitment to confidentiality, professionalism, and trauma-informed, person-centered services
Preferred:
- Associate or bachelor's degree in Social Work, Human Services, Public Health, or a related field
- Experience in housing navigation, healthcare navigation, social services, or case management
- Experience supporting vulnerable or underserved populations
- Knowledge of housing systems, healthcare resources, community programs, and supportive services
- Familiarity with Enhanced Care Management (ECM) and California Alliance for Health (CCAH) program requirements
