About the Role

Das erwartet Dich

Caspar Health connects digital rehabilitation with personal therapeutic care. We help people receive care during and after their clinic stay, wherever they are.

We are looking for a Senior Software Engineer to join our Integration Squad. You will develop, roll out, operate and maintain healthcare integrations across Health Level Seven version 2 (HL7 v2), REST application programming interfaces (APIs), Fast Healthcare Interoperability Resources (FHIR) and CSV imports. You will strengthen our backend engineering capacity and help clinic IT teams and software partners connect reliably to Caspar Health.

The role covers both existing interfaces and new integrations, from implementation and partner onboarding through production support and ongoing improvement. Hands-on experience implementing and operating FHIR integrations in production is a required qualification.

We would like to hear about the production FHIR integrations you have delivered, the systems you connected, the decisions you owned and how you kept the integrations reliable after launch.

This role is located in Germany and requires full physical presence within the country. Due to our existing payroll and social security framework, permanent remote employment from abroad is not supported. Candidates must be eligible to work in Germany and resident from their first day. Please be advised that we cannot offer visa sponsorship or relocation assistance at this time.

Deine Herausforderungen

  • Develop, roll out, operate and maintain integrations across our interface landscape

  • Take integrations through requirements clarification, data mapping, implementation, partner testing, go-live and ongoing support

  • Maintain and improve existing interfaces, resolve production issues and support changes in clinic and partner systems

  • Build reliable data flows for patient information and therapy schedules

  • Design and maintain backend services and versioned APIs in our Ruby on Rails and TypeScript environment

  • Handle duplicate messages, delayed or out-of-order updates, cancellations and partial failures with clear retry and reconciliation behaviour

  • Extend our developer portal with runnable quickstarts, accurate API references, synthetic examples and troubleshooting guidance

  • Work with clinic IT teams and software partners on tested interface contracts and repeatable onboarding

  • Improve automated contract and end-to-end tests, monitoring and operational runbooks

  • Share maintenance, support and knowledge through pairing, design discussions and code reviews

  • Apply secure authentication, authorisation, tenant isolation and data-handling practices with our infrastructure and privacy specialists

Dein Profil

  • Substantial hands-on experience delivering FHIR integrations used in production, including personal responsibility for implementation and troubleshooting after launch

  • Practical knowledge of FHIR resources, profiles, implementation guides, identifiers, terminology bindings, extensions and validation

  • Hands-on production experience with Ruby on Rails, including maintaining and troubleshooting backend APIs and asynchronous integrations.

  • You have practical experience working with AWS infrastructure, including the ability to integrate and manage cloud-based services

  • Experience with healthcare data models, patient and encounter lifecycles, identity matching and appointment changes

  • Strong SQL and PostgreSQL skills, including data modelling, migrations and performance-conscious queries

  • Experience designing secure REST APIs and asynchronous or event-driven integrations

  • Clear documentation and communication with engineers, product colleagues and integration partners

  • A collaborative, pragmatic approach and strong written and spoken English
